ALL OF THE FOLLOWING DOCUMENTS/Persons MUST BE PRESENTED BY PARENT AT TIME OF REGISTRATION
The Student
Child's Birth Certification or Passport
List of Immunizations (Vaccines)
Photo Identification of parent who is present at the time of registration
Lastest Report Card or Transcript (Grades 1-5)
Child's IEP (if student has Special Education Services)
Physician's Health Exam form
Two of the documents below verifying proof of address in the parent’s name within 60 days
Lease agreement, deed, mortgage statement for the residence
A residential utility bill (gas or electric) in the resident's name issued by a utility company(such as National Grid or Con Edison)
A bill for cable television services provided to the residence; must include the name of the parent and the address of the residence.
Income tax form for the last calendar year
Official NYS Driver's License or learner's permit, which has not expired Official payroll documentation from an employer issued within the past 60 days, such as a paystub with home address, a form submitted for tax withholding purposes, or payroll receipt
Voter registration documents, which include the name of the parent and the address of residence
Unexpired membership documents based upon residency (such as neighborhood resident’s association), which include the name of the parent and the address of residence
Under the McKinney-Vento Act, & Chancellor's Regulation A-780, a student in temporary housing has the right to not be denied immediate school enrollment because of their temporary housing status or they lack enrollment documentation.
